In practice
The shipper controls the whole container from stuffing to unstuffing. Rates are quoted per box (20', 40', 40'HC) regardless of how full it is. Free time for demurrage and detention runs per container.

Watch out
FCL does not mean the container is physically full. A half-empty box booked as one unit is still FCL.
